Are you talking about where the hub connects to the ball joint on the lower control arm, or the nut on the end of the driveshaft? If it's the former then it's just the 22mm nut and the split pin. If...

I doubt it's coming from the join. My money is on of the seals: driveshaft, inputshaft, selector rod.
Check that the latch ring at the bottom is tight in it's mounting plate. I've seen one wear loose and cause rattles.

I'd say it's ok because the flywheel is located on the crank by the centre hole. I'd do the bolts up until they start to bite and give the wheel a clockwise twist/bash so the bolts are hard up...
No. The only problem I can think of is the CV stub/drive shaft might poke out a bit far preventing the centre cap to be fitted. I can't run centre caps on some of my wheels for this reason.
